How can hospice help? What services does hospice provide?
Armonia Home Health & Hospice is committed to the total care of any patient facing a life-limiting illness, care for their family and caregivers, and the conservation of healthcare resources.
Here are four ways hospice care can help:
Care at Home
To be eligible to receive hospice services, a patient must be admitted to the program with a certification provided by the physician, based on certain admission criteria.
Hospice care is delivered within a coordinated team approach to reduce or reduce symptoms and improve quality of life. Treatment modalities are determined in consultation with the attending physician.
Pain and Symptom Management
Our hospice nurses and physician are well-versed in the latest pain management options and the best delivery modalities to manage pain and control other symptoms. Our nurses are skilled in assessing and managing pain symptoms while also allowing patients to remain as pain-free and alert as possible.
Bereavement Care
Our bereavement program incorporates several resources that have been proven very effective. As part of our hospice care, we offer individual counseling at the time of death, follow-up visits, phone contact, and more, as needed.
Caregiver Tips
At Armonia, we understand that life can get hectic, especially when caring for a loved one with a life-limiting illness. Our skilled staff can provide helpful tips on routines and consistency, medications, nutritional needs, and information on what to expect at every stage.
When the time comes, our hospice team will coordinate and provide the individualized services you need. With Armonia, you are never alone.
